Submission

 

The following components are necessary for a complete Project Proposal:

  • Completed Project Proposal Workbook
    • Cover Sheet
      • DUNS Number must be completed
      • Project Proposal Type must be: "Tulare Operational Area" for the Project Proposal Meeting
      • Whether your agency is able to accept partial funding for proposed projects
      • Completed Project Proposal Checklist
      • Authorized Agents must be completed
    • Project Overview
      • Choose appropriate Investment Justification(s) and type in appropriate Objective(s) from the dropdown for each of your projects
      • Choose an appropriate National Preparedness Goals (NPG) Missions Area from the dropdown for each of your projects
      • Choose an appropriate National Preparedness Goals (NPG) Core Capability from the dropdown for each of your projects
      • Indicate whether your proposed project type (e.g. radio communications) has been funded by SHSGP in previous funding cycles: if yes, indicate "Sustain", if not, indicate "Build"
      • Select your project's Focus from the dropdown menu
      • Summary/description of proposed project(s) must be provided in the respective Project Overview cell(s)
      • Narrative articulating the Need for the proposed project(s)
    • Solution Area tab(s) (e.g. Equipment, Training, et al.) must be completed. For instructions, please refer to the respective instructions pages listed on Project Proposals page
    • Signed Cover Sheet; options:

Once the above is received, and your Project Proposal is reviewed for completeness by OES, you may choose an available presentation time from the Project Proposal Meeting page.

PowerPoint presentations may also be sent to OES in advance for incorporation into the Approval Authority's Project Proposal packets.
